Severe = I can’t get out of bed and need help taking care of my personal needs.

Moderate/Severe = I am housebound. I can do light housekeeping.

Moderate = I can drive, go shopping, and do some part-time work.

Moderate/Mild = I can work part time and do some socializing.

Mild = I can work full time. My social life is curtailed, and I have to be careful not to overdo.
